    The importance of quality training and understanding is key to accident prevention, today I used the stop work authority after witnessing a misuse of equipment.

    The crew asked the forklift to push steel cable trays together to align them, the forklift was operating with an extension book attachment then proceeded to use this almost like a plough on the ground to push the trays together, dragging the hook and swivel across the ground.

    The job was stopped and the operator was clueless about the potential damage to his equipment, his first thought was to help the guys, his last thought was damaged equipment, surely that is a failure in his training.
