Is your Lock Out, Tag Out program missing a vital component?
So many companies we speak to and assist are missing a vital component in their lock out, tag out programs, TRY OUT!
You can have a dual powered machine, energy retained in the system or even isolated the wrong piece of equipment by mistake. Without that vital try out step you may never know until it is too late.
Every program should be reviewed on an annual period to ensure it is still fit for purpose and compliant, if you have an incident during this period then the JSA, Risk assessment and any procedure must be reviewed to find the failure that failed to stop the incident occurring.
Just like dropped objects, too many people are being hurt through outdated or none existent risk assessments and training on lock out tag out.
